Glikoz metabolizmasının bozulması sonucu oluşan tip 2 diyabet hastalığının
tedavisinden kullanılan etken maddelerden biri olan pioglitazon (PIO) çağımızda en
çok tercih edilen diyabet ilaçlarının başında gelmektedir. PIO hakkında birçok yararlı
bilgi olmakla birlikte PIO içeren ilaç kullanan hastalarda mesane kanseri insidansının
arttığına yönelik iddialar bulunmaktadır. Yapılan in vivo ve grup çalışmalarında bu
etkisini birçok kez vurgulanmıştır. PIO’un insan mesane kanseri riskini artırıp artırmadığını incelemek amacıyla
hücre kültürü temeli üzerine kurduğumuz bu çalışmayla tespit edilmeye çalışılmıştır.
Uygulanan metodlarda hTERT-Uro Cell olarak adlandırılan immortal sağlıklı insan
mesane hücrelerini kullanılmıştır. Çalışmamızda kullanılması planlanan PIO’un
insan mesane hücrelerinde (hTERT-Uro) 24, 48 ve 72 saatlik uygulamaları sonucu
hücre canlılığına etkisi incelenmiştir. PIO’un sitotoksik etkisi, canlı ve prolifere
hücrelerin tespitini sağlayan, mitokondriyal aktivite bazlı MTT testi ile, hücrelerin
potansiyel kanserleşme durumlarını da Transformasyon (Kontak İnhibisyon) testiyle
ve Soft Agar (Ankoraj Bağımsız Test) ile ölçülmüştür. PIO’un deneyde kullanılacak
dozlarının belirlenmesi ve hTERT-Uro hücre hattı için IC50 değerinin belirlenmesi
amacıyla 1, 2.5, 5, 10, 25, 50, 100, 250 μM’lık (sırasıyla PIO dozları) 8 dozluk bir
skala ile mtt testi gerçekleştirilmiştir. Bu deneyler sonucunda, 72 saat ugulamada
IC50 değerleri PIO için 44 μM olarak belirlenmiştir. Ayrıca bu testler sonucu
III belirlenen PIO dozlarının 10, 20, 40 μM’lık 3 dozu kontak inhibisyon ve ankoraj
bağımsız testlerinde kullanılmıştır. Deney verilerinin de PIO’un hTERT-Uro hücre
hattında ve kullanılan deney şartlarında dozların artışına bağlı olarak sitotoksik etki
gösterdiğini ayrıca kontak inhibisyon ve ankoraj bağımsız testlerinde hücrelerde
kanserleşmeye neden olduğu gösterilmiştir.,
Pioglitazone (PIO), which is one of the active substances used in the
treatment of type 2 diabetes caused by disruption of glucose metabolism, is one of
the most preferred diabetes drugs in our age. Although there is a lot of useful
information about PIO, there are claims that the incidence of bladder cancer is
increased in patients using PIO-containing drugs. This effect has been emphasized
many times in in vivo and group studies.
The aim of this study was to determine whether PIO increases the risk of
human bladder cancer. In the applied methods, immortal healthy human bladder cells
called hTERT-Uro Cell were used. In this study, the effect of PIO on human viability
in human bladder cells (hTERT-Uro) after 24, 48 and 72 hours of application was
investigated. The cytotoxic effect of PIO was measured by MTT test based on
mitochondrial activity, which enables the detection of viable and proliferating cells,
and the potential cancers of the cells by Transformation (Contact Inhibition) test and
Soft Agar (Anchorage Independent Test). In order to determine the doses of PIO to
be used in the experiment and to determine the IC50 value for the hTERT-Uro cell
line, the mtt test was performed with an 8-dose scale of 1, 2.5, 5, 10, 25, 50, 100, 250
μM (PIO doses respectively). As a result of these experiments, IC50 values were
determined as 44 μM for PIO at 72 hours application. In addition, 3 doses of 10, 20,
40 μM of PIO doses determined as a result of these tests were used in contact
V inhibition and anchorage independent tests. Experimental data showed that PIO
showed cytotoxic effect due to increase of doses in Htert-Uro cell line and
experimental conditions used and also caused cancer in cells in contact inhibition and
anchorage independent tests.
