Gebelerde Oral Glukoz Tolerans Testi Öncesi ve İkinci Saat Kan Ghrelin, Obestatin, Copeptin ve İnsülin Seviyelerinin Karşılaştırılması

Abstract

Amaç: Bu çalışmada 75 gram glukoz ile yapılan tek aşama oral glukoz tolerans testi (OGTT) öncesi açlık ve yükleme sonrası ikinci saat ve test sonucuna göre gestasyonel diabetes mellitus (GDM) tanısı konulan ve konulmayan gebelerin kan numunelerinde ölçülen insülin, ghrelin, obestatin ve copeptin değerleri arasında farklılık olup olmadığının araştırılması amaçlanmıştır. Gereç ve Yöntem: Gebeliğin 24-28. haftası arasında olan 30 gebe kadın ve gebe olmayan 27 sağlıklı kadın çalışmaya katılmıştır. Gebelere tek aşama 75 gram OGTT uygulanmıştır. Gebelerde OGTT öncesi açlık ve 2. saat, kontrol grubunda ise açlık kan numuneleri alınmış ve glukoz, ghrelin, obestatin, copeptin ve insülin ölçümü gerçekleştirilmiştir. Bulgular: Gebelerde 2. saat insülin düzeyi, açlık ve kontrol grubu değerlerine göre anlamlı derecede yüksek bulunurken (p<0.001), ghrelin, obestatin, copeptin düzeyleri arasında anlamlı farklılık bulunmamıştır (p>0.05). OGTT sonrası 2. saat glukoz değeri ≥153 mg/dL olan gebelerin, OGTT öncesi açlık glukoz değeri ≥92 mg/dL olanlara göre copeptin seviyesi anlamlı derecede düşük bulunmuştur (p<0.05). Gebelerde (açlık ve 2. saat) ve kontrol grubunda copeptin ile ghrelin ve obestatin, ghrelinle obestatin arasında, gebelerin açlık insülin ve ghrelin değerleri arasında pozitif anlamlı ilişki bulunmuştur. Sonuç: Çalışmamızın sonuçları ghrelin, obestatin ve copeptinin gebelerle, gebe olmayan sağlıklı kadınlar ve OGTT açlık ve 2. saat kanda yapılan ölçüm değerleri arasında değişiklik göstermediğini ortaya koymuştur.

Aim: The aim of this study was to compare the levels of insulin, ghrelin, obestatin and copeptin measured in the blood samples of pregnant women diagnosed with gestational diabetes mellitus (GDM) according to fasting and second hour after loading, and test results before single stage oral glucose tolerans test (OGTT) with 75 grams of glucose. Material and Method: The 30 pregnant women who 24-28 weeks of gestation and 27 healthy non-pregnant women as controls were included the study. Single stage 75 g OGTT was applied to pregnant women. Before OGTT fasting and second hour after loading blood samples were taken from pregnant women and fasting blood samples were taken in the control group and glucose, ghrelin, obestatin, copeptin and insulin were measured. Results: While insulin levels at 2nd hour were found to be significantly higher in pregnant women as compared to fasting and control group values (p<0.001), however no significant difference was found between ghrelin, obestatin, and copeptin levels between the groups (p>0.05). Copeptin levels were found to be significantly lower in pregnant women with glucose ≥153 mg/dL after OGTT than those with fasting glucose ≥92 mg/dL before OGTT (p <0.05). In pregnant women (fasting and 2nd hour) and control group, there was a positive correlation between copeptin and ghrelin and obestatin, ghrelin and obestatin, and between fasting insulin and ghrelin levels of pregnant women. Conclusions: The results of our study showed that ghrelin, obestatin and copeptin did not differ between pregnant women, non-pregnant women, and OGTT fasting and second hour blood measurements.
