Abstract: | Amaç: Sağlık çalışanlarının yaşadığı şiddet olayının kök nedenleri çeşitli değişkenler
(cinsiyet, yaş, meslek türü, eğitim düzeyi, şiddete verilen tepki, şiddetin tekrarlanması,
birim vb.) açısından incelenmiştir. Bu amaçlar doğrultusunda Ordu Üniversitesi
Eğitim ve Araştırma Hastanesi’nde görevli şiddete maruz kalmış sağlık çalışanı
arasından 30 kişiye ve şiddeti uygulayan 30 kişiye mülakat uygulanmıştır. Sağlıkta
şiddetin kök nedenlerini ortaya koymak, farkındalık oluşturmak, daha fazla çalışma
yapılmasını teşvik etmek ve son bulması için öneriler geliştirmektir.
Gereç ve Yöntem: Araştırmada nitel tarama yöntemi kullanılmış olup şiddete maruz
kalan sağlık çalışanı ve şiddeti uygulayan bireyler ile yarı yapılandırılmış mülakat
tekniği kullanılarak sağlıkta şiddetin kök nedenleri incelenmiştir. Ordu Üniversitesi
Eğitim ve Araştırma Hastanesi’nin hasta hakları birimi ile görüşülerek gerekli bilgiler
alınmıştır.
Bulgular: Çalışmaya katılan kişilerin otuzu şiddeti uygulayan, otuzu ise şiddette
maruz kalan sağlık çalışanı olup 15’i doktor, 10’u hemşire ve 5’i diğer sağlık
çalışanıdır. Şiddeti uygulayanların 11’ kadın, 19’u erkektir. Sağlık çalışanlarından
şiddete maruz kalanların 18’i kadın, 12’si erkektir. Sağlık çalışanlarının %66’sı daha
önce de şiddete maruz kalmıştır. Şiddeti uygulayanların %40’ı daha önce şiddet
olaylarına karışan insanlardır. Hasta-hasta yakınının hastane kural ve işleyişine
uymaması şiddet nedenleri arasında ilk sırada yer almış olup, sağlık çalışanlarının
tamamı şiddet olayını adli makamlara ilettiği görülmüştür. Hasta-hasta yakını
açısından ilk neden sağlık çalışanlarının uyguladığı tedaviyi yetersiz bulmabeğenmeme olduğu görülmüştür.
Sonuç: Sağlıkta şiddetin nedenleri sağlık çalışanlarına sorulduğunda %27 oran ile
hasta-hasta yakınlarının hastane işleyiş ve kurallarına uymaması, %20 ile hasta-hasta
yakınının sıra beklemeden muayene olmak istemesi, %10 ile randevusuz hastaneye
gelme, %20’sinin uygulanan tedaviyi beğenmemesinden kaynaklanmakta, %13’ü
hastanede çok bekletildiklerini düşündükleri için tartışma çıkarmakta ve %10’u ise
kendilerine saygısızlık yapıldığını düşünmektedir.
Hasta-hasta yakını tarafından şiddetin nedeni olarak %35 oran ile sağlık çalışanının
tutum ve davranışlarından rahatsız olma, beğenmeme, %36 oran ile sağlık çalışanının
uyguladığı tedaviden memnun kalmama, %17 oran ile hastanede çok bekletildikleri
için strese girmiş olmalarıdır.
Şiddeti azaltmaya yönelik alınması gereken önlem ise sağlık çalışanı açısından yasal
yaptırımların yeniden düzenlenmesi gerektiği ilk sırada yer bulurken, hasta-hasta
yakını açısından doğru iletişim kurulması gerektiği vurgulanmıştır. Aim: The root causes of violence experienced by healthcare professionals were examined in terms of various variables (gender, age, type of occupation, education level, reaction to violence, repetition of violence, unit, etc.). In line with these purposes, interviews were conducted with 30 healthcare professionals working at Ordu University Training and Research Hospital, who were exposed to violence, and 30 people who perpetrated violence. It is to reveal the root causes of violence in health, to raise awareness, to encourage more work and to develop suggestions for its end. Material and Method: A structured interview will be conducted face-to-face and over the phone, by interviewing the patient rights unit of the relevant hospital, and obtaining the information of the healthcare workers and perpetrators of violence. Results: 30 of the people participating in the study are perpetrators of violence, 30 are health workers, 15 of them are doctors, 10 are nurses and 5 are other health workers. 11 of the perpetrators of violence are women and 19 are men. Of those who were exposed to violence by healthcare workers, 18 were women and 12 were men. 66% of healthcare workers have been exposed to violence before. 40% of perpetrators are people who have been involved in violence before. 40% of perpetrators are people who have been involved in violence before. Non-compliance of the patient-patient relatives with the hospital rules and procedures was among the first causes of violence, and it was observed that all of the health workers reported the violence to the judicial authorities. In terms of patient-patient relatives, the first reason was found to be inadequate-disliking the treatment administered by healthcare professionals. Conclusions: When the reasons for violence in health were asked to the healthcare professionals, it was due to the fact that the patient-patient relatives did not comply with the hospital procedures and rules with a rate of 27%, the patient-patient relatives wanted to be examined without waiting in line with a rate of 20%, a visit to the hospital without an appointment with 10%, and 20% did not like the treatment applied, % 13 of them argue because they think they are kept in the hospital for too long, and 10% think that they are disrespected. The reason for the violence by the patient-patient relatives was being disturbed and disliked by the attitudes and behaviors of the health worker with a rate of 35%, being dissatisfied with the treatment applied by the health worker with a rate of 36%, and being stressed because they were kept in the hospital for a long time with a rate of 17%. While the precaution to be taken to prevent violence is that the legal sanctions should be reorganized in terms of healthcare professionals, it was emphasized that correct communication should be established for the patient and their relatives. |
