Amaç: Bu araştırma kalp pili takılan hastalara Meleis’in Geçiş Teorisine göre verilen eğitimin anksiyete ve depresyon ile öz etkililik düzeyine etkisini belirlemek amacıyla yapılmıştır.
Gereç ve Yöntem: Araştırma “tek kör randomize kontrollü deneysel çalışma” olarak 10 Kasım 2022-10 Kasım 2023 tarihleri arasında yürütülmüştür. Araştırmanın evrenini Ordu Üniversitesi Eğitim ve Araştırma Hastanesi Koroner Yoğun Bakım Ünitesinde yatan ve kalp pili takılan hastalar, araştırmanın örneklemini ise araştırmanın kriterlerine uyan 70 kalp pili takılan hasta oluşturmuştur. Araştırmanın verileri “Hasta Tanıtım Formu”, “Hastane Anksiyete ve Depresyon Ölçeği” ve “Kronik Hastalıklarda Öz-Etkililik Ölçeği” ile toplanmıştır. Deney grubundaki kalp pili takılan hastalara Meleis’in Geçiş Teorisi rehber alınarak 2. gün ve 5.gün eğitim verilmiştir. “Kontrol grubundaki hastalara araştırma sürecinde herhangi bir girişim uygulanmamıştır”. Veriler tanımlayıcı istatistikler, Pearson’s ki-kare, Yates düzeltmesi, Fisher-Freeman-Halton, Fisher’s ecact testi, Bonferroni testi, bağımsız iki örnek t testi, Mann Whitney U testi ve İki yönlü Robust testi ile değerlendirilmiştir. Araştırma için etik kurul onayı alınmıştır.
Bulgular: Deney ve kontrol grubu hastaların “Hastane Anksiyete ve Depresyon Ölçeği” ön test son test farkı ortanca puanları arasında istatistiksel olarak farklılık bulunmuştur (p<0.001). “Kronik Hastalıklarda Öz-Etkililik Ölçeği” alt boyutları “Düzenli Bir Şekilde Spor Yapma”, “Hastalık Hakkında Bilgi Alma”, “Toplum, Aile ve Arkadaşlardan Yardım Alma”, “Doktorla İletişim”, “Genel Hastalık Yönetimi”, “Ev İşlerini Yapma”, “Sosyal/Eğlence Aktiviteleri”, “Semptomlarla Başa Çıkma”, “Nefes Darlığı ile Başa Çıkma” ve “Depresyonu Yönetme/Kontrol” ön test son test farkı ortanca puanları arasında istatistiksel olarak farklılık belirlenmiştir (p<0.001).
Sonuç: Araştırmada kalp pili takılan hastalara Meleis’in Geçiş Teorisine göre verilen eğitimin anksiyete ve depresyon düzeyini azalttığı ve öz etkililik düzeyini artırdığı bulunmuştur.
Aim: This research was conducted to determine the effect of the training given according to Meleis's Transition Theory on the anxiety, depression and self-efficacy levels of patients with pacemaker implantation.
Material and Method: The research was conducted as a "single-blind randomized controlled experimental study" between 10 November 2022 and 10 November 2023. The population of the research consisted of patients who were hospitalized in the Coronary Intensive Care Unit of Ordu University Training and Research Hospital and had pacemakers installed, and the sample of the research consisted of 70 patients with pacemakers who met the criteria of the research. The data of the study were collected with the "Patient Information Form", "Hospital Anxiety and Depression Scale" and "Self-Efficacy Scale in Chronic Diseases". Patients in the experimental group with pacemaker implants were given training on the 2nd day and 5th day, taking Meleis's Transition Theory as a guide. “No intervention was applied to the patients in the control group during the research process.” Data were evaluated using descriptive statistics, Pearson's chi-square, Yates correction, Fisher-Freeman-Halton, Fisher's ecact test, Bonferroni test, independent two-sample t test, Mann Whitney U test and Two-way Robust test. Ethics committee approval was received for the research.
Results: A statistical difference was found between the "Hospital Anxiety and Depression Scale" pretest-posttest median scores of the experimental and control group patients (p<0.001). A statistical difference was determined between the “Self-Efficacy Scale in Chronic Diseases” subscales “Exercising Regularly”, “Getting Information About the Disease”, “Getting Help from Society, Family and Friends”, “Communicating with the Doctor”, “General Disease Management”, “Doing Housework”, "Social/Recreational Activities", "Coping with Symptoms", "Coping with Shortness of Breath" and "Managing/Controlling Depression" pretest-posttest difference median scores of the experimental and control group patients (p<0.001).
Conclusions: In the study, it was found that the training given to patients with pacemakers according to Meleis's Transition Theory reduced the level of anxiety and depression and increased the level of self-efficacy.
